We provide social workers for our patients to undergo psych assessments, counseling, and education for families on treatment plans. Our social workers are here to help patients receive the best resources and support for the rest of their days.
As an essential component of hospice care, bereavement is also included to provide support for anticipated grieving. Our team understands the difficulty of this time and strives to be there for patients and their families in any way.
Hospice aides work directly under our registered nurses to help provide the highest level of care and emotional support for patients and their families throughout the hospice process. They are there daily to help and assist with any situation.
Home Healthcare Services
For those patients who cannot physically make it to our facility or prefer being in the comfort of their own home, we have a team of physical therapists for in-home care. They provide therapy services defined in treatment plans in residential settings. Our PTs work with physicians, case managers, and adjusters.
Speech pathologists help rebuild skills or gain confidence with communication for older individuals who have experienced impairment. Some may seek speech therapy after a stroke, when diagnosed with a neurodegenerative disease, or to improve articulation and fluency in their communication.
We are here to help individuals regain control of their everyday life activities. Our occupational therapists can assist patients with stress management, daily routines, planning, and other cognitive and behavioral conditions within the comfort of one’s home.
Allow our registered nurses to help patients in need within their homes. Our nurses can come directly to the patient and assist with medical assessments, medication management, bowel support, and more. We are dedicated to helping individuals feel comfortable and content again.
